Does "Start chapters on a right-hand page" change my ebook or Word file?

No. The option in Book design, "Start chapters on a right-hand page (print)", added in version 1.9.24, applies to the print PDF only. Ebooks (EPUB and Kindle) have no left and right pages, so they are not changed, and the Word (DOCX) file stays as it was too.
